Interested applicants must use template provided. Letters of Intent are nonbinding, but are required to obtain access to the electronic application submission portal. Currently contracted CCSB Applicants must submit an application by May 14, 2018 for participation in fourth quarter for plan year 2018. New Entrant Applicants must submit an application by April 27, 2018 for participation in fourth quarter for plan year 2018.
